Setting the scene
This is Erik, they are an 18-year-old male.
Erik previously attended an out-of-hours walk-in centre at age 15 years, where they saw an ECP (Emergency Care Practitioner)
At this intervention, Erik was initially hesitant to speak, they have recent self-harm cuts visible on their hands and arms. Erik is known to have eating disorders.

You are on your final placement of clinical practice at the Community Health centre working alongside a General Practitioner who is taking a Video Call/Telemedicine from a patient (Erik) who is reporting general shortness of breath and difficulty breathing at times. Erik is unwilling to go to the hospital: -

Further information made available during triage/assessment
Erik complains of feeling worried frequently without really knowing why. They state that they have always been this way but that this has increased more recently, particularly since a change in leadership at work. During these periods of ‘worry’, they find it particularly difficult to breathe and also tightness in the chest. They also struggle to sleep for long periods and frequently wake through the night.
